I agree to receive nutrition treatment from Alfredo Angione, RD, CSSD, CSCS as an Ontario Registered Dietitian, co-found of SCN2xL. Treatment will involve guidance about health factors within my own control, including diet, nutrition, and related behaviors. It is understood that treatment will be completed via telephone and/or web-based counseling. I acknowledge the need to provide an accurate and truthful online assessment form and accept receiving all nutritional feedback documents via email, telephone, and web-based conferencing.


I understand that Alfredo Angione, RD, CSSD, CSCS is a Registered Dietitian and does not dispense medical advice nor prescribe treatment. Rather, it provides education to enhance knowledge of health as it relates to foods, dietary supplements, and behaviors associated with eating. While nutritional and botanical support can be an important complement to medical care, I understand nutrition counseling is not a substitute for the diagnosis, treatment, or care of disease by a medical provider.

Nutritional evaluation or testing provided in counseling is not intended for the diagnosis of disease. Rather, these assessment tests are intended as a guide to developing an appropriate health-supportive program, and to monitor progress in achieving my goals.

In order to benefit from the treatment prescribed by Alfredo Angione, RD, CSSD, CSCS, I realize that it is important to inform Alfredo of any changes I make in the application of my diet. It is my responsibility to report any side effects or problems immediately and to make the necessary adjustments to my treatment plan with Alfredo Angione, RD, CSSD, CSCS. I will not hold Alfredo Angione, RD, CSSD, CSCS, responsible for any complications that result from my failure to comply with either of the above.

I have agreed to have my Registered Dietitian keep records of our visits and to file these in a secure and appropriate place. Accurate assessment documentation is required to develop and implement the most accurate nutrition program based on the goals I have outlined. I agree to hold Alfredo Angione, RD, CSSD, CSCS harmless for claims or damages in connection with our work together. I understand that it is also a release of potential liability. I understand there will be NO refunds for any reason. A cancellation fee of $60 will be charged if I cancel my appointment without 24 hours notice.
